Imagine you’re headed out of town for your first vacation in a very long time and you get a phone call.

The loved one you care for is having a problem. They need to go to the doctor but the person you have lined up to help care for them isn’t sure where to go, what medications they take, or what their medical history is. Afterall, they have been stable for a while now. You panic and send a bunch of texts and emails each with pieces of the puzzle. You’re stressed and so is the person who is helping out.

What if we told you there is a better way?

What if we offered you an option to keep all the most important information in one place?

Well, now you can, with the Caregiver Information Sheet!

The Caregiver Information Sheet helps you stay organized by providing an at-a-glance view of common helpful information that your family members, friends, neighbors, respite caregivers, and anyone else who might help you and your loved one should know.

How to use the Caregiver Information Sheet:

This Caregiver Information Sheet is designed to provide at-a-glance information for some of the most important information you might need to provide someone helping you care for your loved one.

From emergency information to medication to food and activity preferences, this sheet helps keep this information in an easy to find location.

You might find you need to print updated copies frequently, or you may find that one copy can hang on your fridge for a few months or years.

In addition, we’ve created this Caregiver Information Sheet to be ‘one-size-fits-most’, meaning that while we hope this sheet helps gives you a place to start gathering helpful information, you might find that it doesn’t all apply in your situation, or that you could really use a much more comprehensive document.

That’s okay! We do hope to provide additional resources in the future, so if you have ideas for other resources that would be helpful to you in your caregiving journey, be sure to drop us a note.
