This Caregiver Information Sheet is designed to provide at-a-glance information for some of the most important information you might need to provide someone helping you care for your loved one. 

From emergency information to medication to food and activity preferences, this sheet helps keep this information in an easy to find location.

You might find you need to print updated copies frequently, or you may find that one copy can hang on your fridge for a few months or years.

In addition, we’ve created this Caregiver Information Sheet to be ‘one-size-fits-most’, meaning that while we hope this sheet helps gives you a place to start gathering helpful information, you might find that it doesn’t all apply in your situation, or that you could really use a much more comprehensive document.
